Choose a patient from your clinical setting that has an alteration in one or more of the systems covered this semester. Introduce the patient by discussing how he/she is altered when compared to normal functioning. Describe why this is (what is the pathophysiology of this alteration). This entry should be typed, double spaced, Times New Roman font, size 12, and 350-400 words.
Journal Entry 2: Data Collection (5 Points)
Using the same patient from the first entry, identify pertinent data collection findings that correlate with this alteration (labs, vital signs, subjective & objective data collection findings, etc). Describe how these findings are reflective of the patient’s alteration. If you cannot find the pertinent data in your patient’s chart, include what you would anticipate to see with this alteration.
